Amazing young man

Hi I am looking for some tips or advice my 12yr old grandson is highly intelligent and is predicted GCSE grades 6-7 - no problem you may think - he has behavioural issues more so around emotional expression which leads him to get easily frustrated angry then poor behaviour, his school are as helpful as they can be, he has just been assessed and we've been told his reading level is that of a 6yr old
For me its a hard one to understand as his work rate understanding etc functions at a much higher level.
We need to find ways to help him progress with his reading as without the reading it will become increasingly difficult for him to progress with higher level work expected from his current year 8, I know there is no easy fix but would appreciate any help advice or tips so I can help this amazing young man reach his full potential
